Recognising critical control points
Sometimes situations can sneak up on you and you wonder how you ended up here. A number of ex politicians are probably still thinking that. One thing they failed to recognise was a critical control point where they could act to change the outcome.
There are plenty of examples of bad outcomes resulting from a failure to recognise a situation and act at a critical point – lots of VFR into IMC accidents, unstable approaches, chemical drift incidents, takeoff accidents and failed policy-free election campaigns to name a few.
