Investigators probing the June 2023 crash of VH-DAW, a Cessna 310 flown by Broome Aviation, uncovered multiple reports to the regulator from former passengers and employees which appeared to detail poor management and unsafe operational practices.

Starting on 17 June, the flights will shift to Brisbane to “streamline operations” in a shared service with Sydney and Melbourne. The 747-8F has a cargo uplift of 135 tonnes per flight, complementing Cathay’s existing cargo capacity in passenger aircraft.

In a damning report, the ATSB said complaints were received by CASA against Broome Aviation from both pilots and a passenger, one of which stated the mentality of the owner and manager wasn’t “conducive to safe aviation practices”.

VH-UZI's approach became unstable after the first officer accidentally pressed the wrong button, and the crew did not conduct a go-around, the ATSB found.
