If idustry actually relied on government and regulation to drive safety, the history of aviation would look very different indeed.

While our various regulators are often still in the dark ages in their approach to safety and certainly how it manifests in regulation, it is always incumbent on individuals in industry to continually strive to identify and mitigate risk, to robustly analyse past performance and incidents and to research and action ways to do better.
