Finger trouble, human error, a breakdown in communications, call it what you like but it is still the primary reason why most aviation accidents occur. The recent USAir 737-400 overrun into New York harbour is indeed a classic case of not only procedural but also communication and command breakdown being the primary villain in an accident that was just waiting to happen.
