Too Hot, Too High & Too Short
The Convention on International Civil Aviation held in Chicago, U. S. A. in 1944 led to the formation of the International Civil Aviation Organisation (ICAO). Subsequently, an Accident Investigation Division was formed and international standards and recommended practices for accident investigation were adopted.
Australia, as a signatory to the Convention, discharges its responsibilities through the Bureau of Air Safety Investigation. The Bureau investigates all accidents and incidents involving civil aircraft operating in Australian Flight Information Regions. This includes regular public transport, general aviation and sport aviation activities. The Bureau can also be involved in overseas investigations of accidents involving Australian-owned, operated or manufactured aircraft.
The objective of accident and incident investigation is accident prevention and does not include the apportionment of blame amongst those involved.
