The pilot of a Beechcraft B200 Super King Air that crashed shortly after takeoff had the rudder trim in a wrong position at takeoff, the Australian Transport Safety Bureau says.
A Virgin Australia Boeing 737-800 came within five metres of leaving the runway after landing at Christchurch Airport because pilots did not configure the aircraft properly for a wet arrival, the Australian Transport Safety Bureau (ATSB) says.
The Australian Transport Safety Bureau has issued a safety notice after a Malaysia Airlines A330-300 took off from Brisbane with its three pitot probe covers still on.
The ATSB says current CASA legislation does not require commercial operators of aircraft up to 5,700kg in weight to provide instructions and procedures for crosschecking fuel quantities before and during flight.
The ATSB has opened an investigation into a depressurisation incident on board a Qantas Freight 737 freighter that led to one of the pilots becoming incapacitated.
